Job Description:
The Comprehensive Care Clinics at Henry Ford Health are looking
to expand to additional clinic sites within the organization.This
clinic structure is distinctly different than typical primary care
clinics.Henry Ford Medical Group developed the Henry Ford
Comprehensive Care Clinic (CCC) to better meet the needs of a high
risk, complex patient population with chronic disease. This model
offers timely, intensive, ambulatory care services; improving
overall health, preventing exacerbations of chronic illness driving
costly utilization including hospitalization and emergency room
use. This value based/patient centric model consists of a
multidisciplinary team of providers and care managers understanding
the special needs of this population creating value and enhancing
quality of life.Duties include but are not limited to:CCC
Physicians will lead a team of other professionals in the care of
their patients in smaller panels of 200 - 300
patients/physician.Physicians will provide ambulatory longitudinal
care to patients during a regularly scheduled clinic Monday-Friday
Appointments are schedule at minimum every three months for 30-60
minutes to give you time to comprehensively review and optimize the
patient as a whole. Patients will also have touchpoint with a team
member monthly.Due to patient complexity, there is some after-hour
and weekend calls shared among the group.In addition to clinical
contact time, the CCC physician will participate in weekly Case
Conferences and quality improvement meetings to insure optimal
outcomes for CCC patientsPharmacist support will be provided to
improve patient medication education, support adherence, and
minimize side effect profiles. Case Management and Community Health
Worker support for any social needs.Home care services and
community care workers will augment CCC services to provide
additional support for patients in their homes.Essential
Knowledge/Skill Sets: Strong clinical experience in the ambulatory
setting with interest in focusing practice in the treatment of
patients with several chronic diseasesExperience in managed health
care and community-oriented primary care, preferably in an
underserved environmentEnjoys working collaboratively with a
multi-disciplinary team including nursing, pharmacy, case
management, etc. to provide the best and safest patient
care.Ability to develop more clinical role responsibility in care
of a complex patient including basic subspecialty management and
consultative collaboration streamlining non-essential, costly
services. Ability to partner with inpatient and emergency medicine
physicians in lessening use of hospital-based services unless
neededInterest in development of growth of new model of care within
HFMG that supports population managementDevelop ability to remotely
manage patients at home, or manage some mild exacerbations of
chronic illness in the clinic settingFoster a physician culture of
collaboration, transparency, and accountability for high-quality
care with comfort sharing data in process improvement Develop an
awareness of medical risk models of care, assuming greater overall
ownership of patient outcome.Required Education/Experience:Licensed
or eligible to be licensed as a physician in the state of
MichiganBoard certification in internal medicine or family
medicine. 5 years' experience in clinical practice
