Job Description Job Description The RN Infection Preventionist
(IP) & Wound Nurse is responsible for identifying, investigating,
monitoring, and reporting healthcare-associated infections & Skin
Management Program. The IP & Wound Nurse collaborates with teams
and individuals to create infection prevention strategies, provide
feedback, and sustain infection prevention strategies. $7,500
Sign-On Bonus ESSENTIAL DUTIES AND RESPONSIBILITIES Which must be
performed in order to carry out the position purpose summarized
above (general representation of the key duties and
responsibilities of this position; other duties may be assigned, as
required): · Identify and maintain effective methods to gather
infection prevention and control related information. Coordinate
and monitor monthly departmental infection control and process.
Collect, analyze and provide infection & wound information on
in-house infections & skin alterations to nursing staff and other
healthcare practitioners. Investigate and identify control
procedures and risk minimizing measures, including employee health
transmission and risk. Investigate and identify control procedures
and risk minimizing measures for skin alterations preventions &
wound development . Monitor appropriateness of precautionary
procedures for individual patients. Maintain records of infections
& wounds, procedures, outcomes related to infections , skin
alterations and risk assessment, prevention and minimizing
activities. Coordinate completion of ongoing immunization tracking
activities and implement measures to improve compliance, including
influenza, pneumococcal and tuberculosis. Coordinate completion of
ongoing wound tracking activities and implement measures to improve
compliance. Complete weekly wound rounds with specialty wound
service provider. Assess new and or readmissions, & newly developed
skin alterations and implement the skin management protocals. Care
plan development and management for infections and skin
alterations. Participate in care conferences as designated for
infection & skin collaboration. Participate in clinical and risk
management meetings. Review infection and prevalence rates monthly
providing recommendations to the Infection Control or Quality
Assurance and Performance Improvement Committee. Recommend
education for employees and other healthcare practitioners to
minimize, prevent, and control infections. Educate newly hired
center staff on infection prevention and control techniques
including hand hygiene, blood borne pathogens, PPE utilization and
sharps handling. Collaborate with administrator, medical director
or his/her designee, DON and department managers on infection &
skin prevention and control protocols and practices. Collaborate
with Human Resources Director on exposure and control plan and
follow-up. Evaluate availability of materials and equipment for
infection & skin prevention and control. Coordinate
outbreaks/trends investigations with the director of nursing and
medical director. Monitor changes in state and federal regulations
and current industry-standard practices. Contact state and local
health departments for specific regional guidelines, such as
tuberculosis, outbreak investigations, etc. Provide monthly,
quarterly and annual reports for the Quality Assurance and
Performance Improvement Committee. Complete annual tuberculosis
risk survey. Prepare recommendations for changes in center practice
to the Quality Assurance and Performance Improvement Committee.
Partners with the facility medical director, DON and consultant
pharmacist to implement and maintain a antimicrobial stewardship
program to ensure appropriate antibiotic utilization. Provide the
people we serve with clear opportunities to direct their own lives
and decide how they will spend their day. Ensure a safe environment
for residents and utilize tools and equipment in a safe manner.
Report all hazardous conditions including incidents, injuries, and
equipment to the appropriate Supervisor immediately. Adhere to
mission statement and all company policies including, but not
limited to job description, HIPAA, Residents’ Rights, annual TB
test, and education assignment completion. Participate on
committees as assigned. Adapt to changes in responsibilities in a
positive manner, be receptive to new ways of completing tasks, and
flexible in carrying out assignments. QUALIFICATIONS A. Specific

Certified. Successful completion of the CDC Infection Preventionist
Course Demonstrated understanding of related local, state, and
federal regulations. Leadership skills; training, coaching, team
building, decision making and problem-solving skills. In-depth
communication skills: demonstrated ability to present material
orally and in writing in an articulate and concise manner;
demonstrated ability to listen and process others’ points of view.
In-depth emotional intelligence: behavioral maturity, demonstrated

self-awareness and accurate self-assessment; demonstrated ability
to prevent, reduce, and resolve conflict; sensitivity and empathy,

Microsoft Office Suite; PointClickCare charting. Ability to manage
multiple, complex priorities with demanding timeframes, strong
