positions available on all shifts at Judson Manor MUST HAVE ACTIVE
STNA/CNA TO APPLY AND MUST HAVE ACTIVE BLS AND ACTIVE FIRST AID
CERTIFICATION TO APPLY General Purpose: Perform a variety of
supportive resident care duties. Provide related services that
promote comfort, safety and meet the residents’ personal needs.
Essential Duties and Responsibilities: Assists residents with
activities of daily living (ADL) according to guidelines and member
care plans, which includes but is not limited to Bath/Shower, Mouth
Care, Hair Care, Toileting, Skin Care, and Nail Care. Implement
fall prevention program for any resident at risk for falling
Monitor that each resident’s anti-fall device is in place and
activated. Report any broken/non-working devices to
nurse/supervisor immediately. Report any fall risk behavior to
nurse (such as resident attempts to get up without assistance).
Report any changes in resident’s condition/appearance/function to
nurse or supervisor Call light procedure Assure that all residents
have access to call light at all times and know how to use it.
Respond to all call light/paging system promptly, courteously and
provide necessary assistance Perform selected treatments according
to policy and procedure. TX’s include: Application of TED hose.
Care of TED hose, washing daily. Vital Signs Obtain weights as
ordered in timely manner, demonstrating ability to take (weights)
accurately. Mechanical lift use competency with the use of two
persons when using. Bathtub competence in use and cleaning after
each use. I & O obtain as ordered with accuracy of measure and
documentation. Transfers using safe techniques. Remove foot rests
from w/c before transferring to and from using appropriate number
of persons to perform transfer according to plan. Apply braces and
sling as ordered, correctly and in a timely manner. Oxygen use
demonstrating proficiency in use of O2 systems – mask and cannula.
Know and maintain liter flow ordered by MD. Demonstrates competency
of filling portable O2 to Ensure adequate amount of O2 available to
resident at all times. Appropriately uses concentrator when
resident is in room. Monitor and inform nurse if humidifier bottles
on concentrator are low. Assist resident to maintain optimal
nutritional and hydration status according to guidelines and member
care plans, for mealtimes and nourishments. Document resident
intake in POC after each meal and nourishments. Notify nurse of
poor intake of 50% or less consumed at each meal as well .
Infection control: Follows all isolation protocols including hand
washing, linen storage, and soiled linen procedures Admission
procedure: Prepare room. Make sure bed is made. Necessary equipment
present ie: phone, TV remote, lights functioning Obtain weight and
vitals. Obtain w/c and admission supplies. Make resident
comfortable in bed or w/c. Puts all residents’ belongings in
appropriate drawers/wardrobes. Complete personal belongings
inventory sheet Maintain neatness of resident room and all work
areas: Performs residents laundry in timely manner: Ensures that
all clothing is labeled, stored and maintained appropriately.
Informs manager/social worker when resident needs clothing Reports
worn clothing to supervisor. Completes all documentation in a
timely manner including: Admission paperwork. Resident care flow
sheet. BM record. Dining intake record. Restorative record. Bath
audit. Maintains dignity of each resident at all times. Knock and
announce prior to entering resident room Address resident by
preferred name not nickname Provide resident your name and reason
why you are there prior to providing care Close door or bathroom
curtain for privacy Provide residents with meaningful activity:
Participates in identifying residents’ daily pleasures. Assists
residents in seeking and engaging in their individual daily
pleasures. Provides opportunity for intimate social gatherings in
resident neighborhoods daily. Empowers self to actively seek
avenues for residents to become involved and engaged in their
community. Maintains confidentiality regarding resident
information. (HIPAA) Participates in planning and evaluating the
interdisciplinary care plan through one-to-one interaction with
Resident Care Manager and through participation in the resident
care conference. Miscellaneous : Maintains a record of acceptable
attendance and punctuality. Maintains twelve hours of continuing
education annually. Observes all company rules and safety
procedures. Uniform compliance Other duties as assigned. Judson

with. Minimum Qualifications, Education and Certifications: High
school diploma. Must be a graduate of a state approved nursing aide
training program (with plans to take the mandated test for
certification within 30 days of offer) or has current certification
as a state tested nursing assistant/certified nursing assistant
STNA/CNA. Computer literacy required. Physical Requirements: The
physical demands described here are representative of those that
must be met by an employee to successfully perform the essential
functions of this job: Physical demands may include kneeling,
climbing ladders, reaching, pushing, manipulating, talking,
hearing, standing for extended periods, crouching, stooping,
walking, lifting up to 40 lbs., transferring resident up to150 lbs
with assistance, if necessary. Working Conditions: Clinical
environment: General clinical environment including; resident
units, resident rooms and care areas. May have exposure to resident
waste and blood products and will use PPE (Personal Protective
Equipment) and universal precautions accordingly. May be exposed to
infections, pathogens and will follow isolation precautions.
Associate may be exposed to combative residents and/or residents
with delirium/confusion
