Encompass Health Corporation is seeking a dedicated Certified Case Manager to join our expansive team, providing top-tier care management within our diverse network of inpatient rehabilitation hospitals, home health agencies, and hospice agencies. The ideal candidate will specialize in helping patients understand their health status and coordinating long-term care plans that align with patient needs and compliance standards. This role requires a robust understanding of healthcare procedures and patient support, ensuring the development of care programs that enhance patient outcomes and satisfaction. Evaluate patient needs and develop comprehensive care plans in collaboration with healthcare providers. Facilitate multidisciplinary team meetings to ensure seamless communication and cohesive patient strategies. Monitor patient progress and adjust care plans as necessary. Provide patients and families with understanding of health conditions, medical options, and resources. Ensure compliance with healthcare standards and ethics in care management. Document all patient interactions and interventions in patient care management systems accurately. Excellent communication and interpersonal skills Strong organizational and critical thinking capabilities Proficiency in care management software and patient databases In-depth knowledge of healthcare regulations and accreditation standards Medical Insurance Dental Insurance Vision Insurance Life Insurance Certified Case Manager (CCM) credential required Licensed Registered Nurse (RN) preferred
