well-being. We have an exciting opportunity for an Intake Hybrid
Therapist to join our Intellectual and Developmental Disability
(IDD) team of talented professionals. This position comes with a
$3,000 sign-on bonus. POSITION SUMMARY The Hybrid Intake Therapist
is responsible to provide a welcoming experience and conduct a
thorough and comprehensive assessment with adult, adolescent and
children with intellectual and/or developmental disabilities as
well as co-occurring mental health and/or substance use, in an
effort to determine service eligibility and accurately identify
service needs and preferences. The Hybrid Intake Therapist will
also use their work time to assist with completion of
biopsychosocial assessments, carry a small caseload, and complete
other services upon the direction of the Program Manager. Services
are delivered primarily in the office setting on a scheduled or
walk-in basis. On an infrequent basis there may be a need to
complete services in the community. Full-time staff must work 37.5
hours per week, including a minimum of 4 hours per week after 5:00
p.m. RESPONSIBILITIES INCLUDE Conduct intake assessments to
identify areas of need and service preferences and complete
documentation within 48 hours; Gather and integrate information
from all available sources at the time of intake, including
consumer self-reports, reports from family members, guardians and
documented medical and treatment history, school IEP’s, needs
surveys, assessments from other disciplines, etc.; Utilize
motivational interviewing techniques to assess and articulate the
motivation of consumers to address specific needs identified during
the assessment process; Provide consumers, family members,
guardians and support coordinators with complete and accurate
information regarding each of the services and supports available
to meet needs identified during the assessment process; Assist
consumers with initiating the person-centered planning process
(consistent with principles of Person-Centered Planning,
Self-Determination and current Medicaid Guidelines); Maintain a
partial caseload and provide therapy services to assigned
individuals and families Develop Safety Plans and/or crisis plans
as circumstances require based on needs identified during the
assessment process Develop Positive Behavior Support Plans as
needed Identify those consumers who require more extensive
evaluation to identify their needs and preferences and facilitate
referrals for psychological testing, therapy services including
groups, etc. as needed; Participate in identified scheduled
clinical supervision, case consultations and team meetings;
Document all face-to-face service activity and phone contact
pertaining to consumers; Assist with completing required
documentation within the I/DD service community as needed Perform
within acceptable standards of identified productivity for the
position, consisting of intakes, assessments, client caseload and
biopsychosocial assessments. Respond in writing to routine chart
reviews and audits; Other duties as assigned. Requirements A
Master’s degree in Social Work, Counseling or Psychology, from an
accredited college or university. Applicable licensure issued by
the State of Michigan is also required TLLP, LLPC, or LLMSW as well
as LLP, LPC or LMSW. Must be able to meet the criteria within 120
days of hire for Children’s Mental Health Professional, Qualified
Intellectual and Developmental Disability Professional and have at
least eight (8) months experience providing mental health treatment
services to adults and/or children with an intellectual and/or
developmental disability. Additional experience providing services
to those with mental illness preferred. Valid driver’s license and
favorable driving record is required. Must have the ability to
engage and develop therapeutic alliances with adult, adolescent and
children with intellectual and/or developmental disabilities, their
families and supporters. Must be an expert in helping consumers
identify their service needs and preferences, formulate diagnoses,
complete biopsychosocial assessments, initial treatment plans, and
establish services based on their treatment plans. Have the skill
set to be able to determine eligibility for services as a person
with an intellectual and/or developmental disability, per the
federal guidelines. Additional requirements include a nonjudgmental
approach that reflects cultural competency and sensitivity to the
unique needs and diverse experiences of adults and children with
intellectual and/or developmental disabilities, as well as
co-occurring mental health and/or substance abuse/dependence, their
