Job Description:
The position of Guardian Representative consists of assisting
the Guardian with making arrangements for the care and custody of
the client (an impaired adult with traumatic brain injury, spinal
cord injury or dementia) by making sure their daily needs are met
and they are getting the care they need. We need someone who is an
effective communicator, incredibly organized, and self-motivated.
Our ideal applicant will have experience in social work and two
years of experience working with traumatically brain-injured
individuals or developmentally disabled individuals.
Responsibilities:
--- Make decisions using best judgment in key areas of client's
life, and follow through on decisions made for wards such as living
arrangements, medical decisions, quality of life improvements,
etc.
--- Manage and maintain a case load of clients and complete all
required documentation under the supervision of the Director of
Guardianship Services
--- Make visits to clients within and outside their home setting
(i.e. nursing home, group home, independent living, assisted
living, apartment, house, etc).
--- Coordinate and/or attend inter-disciplinary team care
conferences
--- Attend medical appointments with the clients
--- Make sure that clients are receiving the care outlined in their
plan of care
--- Participate and attend required company meetings
--- Maintain confidentiality
--- Assist with other special projects, perform additional
assignments as required
Qualifications:
--- High school diploma or equivalent. College education preferred.
Social work degree would be very helpful.
--- 2 years experience working with traumatically brain injured
individuals or developmentally disabled individuals.
--- Valid Michigan Drivers License
--- Highly proactive with the ability to manage multiple priorities
and use independent judgment.
--- Strong analytical and organizational skills with requisite
follow through and high quality work.
--- Excellent communication skills both verbal and written; ability
to interact credibly with professionals at all organizational
levels.
--- Creatively initiate and implement client plans.
--- Ability to work independently with agility in an evolving
environment.
--- Self motivated and professional.
--- Ability to give full attention to what other people are saying,
and to actively look for ways to help people.
--- Ability to adjust actions in relation to others' actions, and
to manage one's own time and the time of others.
